Target Audience

The educational design of this activity addresses the needs of healthcare professionals who care for people living with HIV, including MDs, NPs, PAs, nurses, pharmacists, case managers, peer educators, medical assistants, and office staff.

Educational Objectives

Upon completion of this program, participants will be better able to:

  • Discuss risk factors and strategies to identify people with HIV who are lost to follow-up
  • Identify effective communication and educational tactics to improve re-engagement among people with HIV who have fallen out of care
  • Collaborate with community organizations to improve re-engagement in care for people with HIV 
  • Incorporate trauma-informed care into practice to retain and re-engage people with HIV in care

Program Description

This grassroots initiative aims to educate clinicians and community partners about the dangers of patients becoming lost from HIV care and strategies to re-engage them. Balancing existing data and clinic models with their own success stories, Drs. Ahonkhai, Hawkins, and Trooskin will present data on consequences of becoming lost to follow-up, identifying patients lost to follow-up, strategies to bring patients back into HIV care, and optimal ways to collaborate with community partners. This event will allow clinicians to network with community partners to form or strengthen relationships that can aid in re-engaging patients in HIV care. Further, resources for both clinicians and community partners can provide additional opportunities for patient education and ancillary services that can assist with care re-engagement.
