WELCOME TO THE BUILDING HIV PATHWAYS™ COMMUNITY RESOURCE BUILDER
Patients with HIV are at risk of falling out of care; clinicians and community partners can work together to help identify these patients and encourage them to return to care. A key component of re-engaging patients in care is addressing their competing priorities. This depends on the strength of the community and relationships between clinicians, patients, and community partners. Resources such as temporary housing or legal services or food banks can help to address patient needs and re-engage patients with HIV in care.
